Cornwall Carers Service is funded by Cornwall Council and Cornwall & Isles of Scilly NHS. It is delivered by Cornwall Rural Community Council, Age UK Cornwall, Carers Rights and Disability Cornwall & the Isles of Scilly. This is a survey for carers in Cornwall. It is important that we receive your feedback as it will enable us to shape and improve our services to support you as best we can in your caring role.

We would therefore very much appreciate it if you could spare the time to complete this survey.
